I've tried to get the star rating to work for a few hours as described at this link but I have not gotten it to work properly (Attached file stars.png). I changed the image to a png and the stars are different but I don't think that's causing the stars for five questions to be on one line.

Steps taken (Attached file all_separate.txt):
1) Created a numeric input question
2) Hide tip = yes
3) Always hide this question = No
4) Clicked on source and added the code for the input and java script
5) Added for additional questions with the same method

It didn't work so I viewed the source of the sample and it looked like someone placed all of the java script in one question and only the input lines in the additional question source. (Attached file - all_java_together.txt)

Using the "Array 5 point choice" question you can set the star rating option without any need for additional modifications.
