Dear Forum,
I would like to design a matrix (taken as matrix with numbers - checkboxes) with a textfield in the last line so people can add their own option. It should look like this:


What I have found until now searching the forum is that it would only be possible with a filter afterwards, asking people to specify the "other activity" option. Is there a nicer way to implement it (closer to the pen&paper version I use as guideline) I am not aware of?
