LimeSurvey output to queXML / the generation of paper forms is now well supported and stable. Just recently some updates where pushed to LimeSurvey to allow for styling the output of the paper form (see: github.com/LimeSurvey/LimeSurvey/pull/344 )

The queXF package can them be used to process the scanned paper forms. This workflow is used in our survey centre and many others that I know of.

What hasn't been maintained recently is the ability for queXF (using LimeSurvey's remote control feature) to send data from completed paper forms in queXF back to LimeSurvey so there is a single database containing responses from both modes (web and paper). This is simply because it hasn't been used in our survey centre and no one else has offered to maintain / sponsor development for it. I'd be happy to assist anyone who wants to contact me about this integration.

It must be possible to do it manually with VV export/import without any dev actually.
