Search for tooltip here in the forum, there are quite a few explanations. And depending on your Limesurvey Version (and it is NOT "Php" - you find the Limesurvey version at the bottom right corner of each admin page) it might be as simple as include some bootstrap code for it to work.
However, you are using a custom template, this could have an impact on if things work or not, depending how much customization was done to the template.

Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
Always provide a LSS file (not LSQ or LSG).
