Not able to add options in a new survey

I recently upgraded from version 3 to the above mentioned version.
Now i am facing problems in adding multiple options in survey!
I get 500 error on ajax call and the flowwing msg
The table "{{tokens_0}}" for active record class "TokenDynamic" cannot be found in the database.
Is this a know issue with this version 5 or i am doing something wrong
I have attached the screen shot of the error
