If it worked fine for 4 weeks and you did not do any update to Limesurvey, you will have to check which changes have been made to your server.
Because a running system does not just stop working like that, without any changes made.

Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
Always provide a LSS file (not LSQ or LSG).
