There is an export of the questionnaire to queXML and from there you could create a PDF version.
However, if you want the questionnaire just to be saved in PDF/Word (usually it goes the other way round for us, we have a questionnaire in Word and develop an online questionnaire based on this, but anyway...), you could switch the questionnaire to all in one and then export it to HTML. You should be able to open this in Word and convert to PDF from there.
Not sure about filters, equations, etc. though.

Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
Always provide a LSS file (not LSQ or LSG).
