I'd like to host some questionnaires and don't want to write it myself. Since Google Forms is not the best privacy-wise, I'd like to self host it. But LimeSurvey – which I used in the past – does take significantly longer to set up than a simple Google Form.
Is there a more minimalistic, FLOSS alternative to LimeSurvey? Should run on PHP and flat file storage if possible.
But I find that simple forms can be set up quite quickly with LS. I don't know what your expectations are, but you should have a simple questionnaire up and running with LS in 10-15min.

Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
Always provide a LSS file (not LSQ or LSG).
