I have one company but multiple departments, each department has a customer satisfaction survey, the first 5 questions in every survey are identical in all surveys and they ask about the company in general, while the rest of the questions in each survey are department specific.
we use those 5 questions to run statistics and measure the satisfactions of our customers company wide and compare between the departments and see where is the lowest score that we need to focus on and the highest score that we need to encourage, etc...
so my question is, is there anyway to create a survey that has 5 questions, and embed this survey in every other survey i create?
But why don't you use the end-url of your first (general) survey to link to the next part?
You said, they are department specific, so you have information about the department, either by a participant attribute or an answer to a question in the first general survey.
Or you create a survey with all questions for all departments and use custom attributes in the token table to show and hide questions according to your needs? I think this is by far the best solution.

Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
Always provide a LSS file (not LSQ or LSG).
