We have strange issue for some reason in dropdown list of surveys under inactive surveys it shows 3 blank items. Not sure what is the cause of it. please see the below screenshot.
You should select these empty items. If these are surveys there might be invalid stuff in the name field.
Without seeing the HTML code it is very hard to diagnose the issue.
What version of LimeSurvey is used?
