Do you have access to the database? Maybe you can inspect the question rows in database table <prefix>_questions?

MySQL? If yes, run this:

SELECT type FROM <prefix>_questions WHERE sid = <survey id>

Replace <prefix> with your database prefix (e.g. "lime"), and <survey id> with survey id.

If any result shows type as empty or null, you need to change it.

Or maybe better: Just visit question 478, and save it again (would overwrite previous type value).

Prefix is not the same as database name. If you run "show tables;", all tables should begin with a prefix. Standard is "lime_".

Sorry, I can't see the error since I don't have access to your installation. :) You can save the error as an HTML file and attach it, if you want to.

Run this database command to find your question:

SELECT * FROM <prefix>_questions WHERE qid = 478;

Similar to re-starting MS Windows it often helps to copy such buggy questions or copy the whole survey. That sometimes fixes such problems automatically.
