In Yes/No, I set the option to NOT show the non answer field, but it's still showing.

How do you turn that off?

That's a bug recently fixed - bugs.limesurvey.org/view.php?id=10554 . It should be fine in the next release but in the meantime you can change the "Display type" to "Radio list" in advanced question settings.
