Just upgraded to Version 2.05+ Build 150310.

Encounter issues with uploading image at the Survey setting.

1. At the KCFinder window, after Upload an image, it seems to be uploaded but the thumbnail appear to be blank.
I then check on server (already ensure /tmp and /upload directories are with writeable permission), the image is not created in the .thumbs folder, but it does appears under upload/images.

2. When click on this image, it returns to the Image Properties window as a broken image in the preview box.
The URL appears to be /upload/images/picture.png
I suspect the URL is wrong, so I tried appending limesurvey/upload/images/picture.png (because our URL is xxx.domain.com/limesurvey.) and it works.
