Hello, I just encountered a problem with Version 3.21.3+191219 that involves loss of data.

Although the attachment is quite self-explanatory, I am going to add a few remarks as to how to reproduce the problem:
1. Create a survey (S) with defaults.
2. Add two groups (G1 and G2).
3. Add one question to each of the groups, namely, Q1 and Q2, which are both of single-choice list (radio).
4. Add an answer option A1 with text "1" to both Q1 and Q2, respectively.
5. Activate S in open access mode.
6. Execute S and choose the only option ("1") for both Q1 and Q2.
7. View the response.

Expected result:
[Q1] = A1[1] and [Q2] = A1[1]
Actual result:
[Q2] = A1[1]

Feel free to request any missing information I fail to mention.

Yes. A1 of Q1 is also missing in the answer table and in an export to csv.

Is there any log file I can refer to in order to diagnose the problem?

Have a look at the change logs of 3.21.4.
There seems to be something about your issue.
