I've "inherited" survey tool from a co-worker and in order to learn I have decided to set it up on local host.
The problem I have which I cannot sort out is that after I apply custom template (exported from original survey) java sript stops working (questions are not shown/hidden based on input from previous questions). With any of the default templates it's OK.
I'm not really sure where to look at first to sort it out.
I know the question is really vague but I would appreciate any tips.

BTW Can you help to identify the template. I guess it's dowloaded not creeated by previous guy.
I have played a bit more and it looks like it's not the template. I have download few other templates and it doesn't work on any. But it works on all default ones.
Also after I change the template to my custom and refresh the survey, question show/hide but template is not fully applied, like some html markup missing, mainly top part missing and pulled to left. I have to reopen the browser and then survey dispalys OK but then show/hide is not working.
