Limesurvey not recognising: Slider + Not applicable design

Hi everyone,

HNY!
I used some of the ideas on the forum and created an array (numbers) question showing a slider (1 to 10) + NA (radio button). I am happy with the formatting and all. The problem I have is that - for some reason - Limesurvey does not save the answers. On each row, the NA is exclusive against the slider. 
When I run it with checkbox layout off, everything looks ok but it does not store the answers and it says some questions were not answered. 
When I run it with the checkbox on, it adds one extra checkbox (which would not be an issue) but it still does not store the answers. If I disable the slider, then it shows dropdown menus in each cell and it saves the answer (one answer per row). 
I attach a minimal example, I'd really appreciate some help on the changes needed to make Limesurvey save the answers and let the respondent past this question.

Hi,
please, do not provide neither lsq (question) nor lsg (group) exports.
Only lss (survey)

Copy your survey, remove everything not related to your issue; send this lss export.

Hi,

this is obvious.
There is no listener on the sliders.
You only insert them, but you do not capture the values when they are moved.

Hi Joffm,

Thank you for your feedback. I added back the listener, somehow I had managed to not include in the code.
It does save the answers now but it does not recognise the code for NA (last column). I replaced the original code 999 with 11 to declare it in the Limesurvey display, it is shown as an option in the html but it is still not recognised as a valid code.
On the side, I guess that leads to the impossibility to make the question mandatory.
If not mandatory, then the slider values are saved correctly. If value NA is chosen (11), then there is nothing saved.
I am sending the updated survey. Any help would be much appreciated.

just one question.
Why do you try do implement this?

Why don't you use the built-in slider with "reset" (here with a changed text)

Or - and in my opinion this is most suitable question type - a simple array.
  

Or you may use a slider with ticks and more styling
 

And there  is this solution - with a real "NA" button, which you may extend to your wishes

My reasons were linked to formatting - beyond functionality.
I've got it working by using the Multiple Numerical Input question type. It's still confusing because the Help instruction reads 'Choose a number between 1 and 11' and there are no numbers on the scale but I am going to just hide that.
