I am trying to add in a "other" option with two text boxes as shown in the screen however my survey displays only one text box and I need the respondents to type in their option if needed and a separate text box for rating that option. How can I do that? Or is there a better way to do it with the array option since I was not able to add a "other" option with text box there as well

What do you really want to do?

When I read the question text of the first screenshot it seems to be a simple ranking question with an "Other" option.
And as you mentioned the "array" option, well, there is this solution
 

Of course you have to validate that there are no doubles.
Easy done by the function "countif". Just make sure that the count of each answer option is less than 2.
 

But in my opinion an "other" option in an array or in a rankoing question is useless.
What will you get?
A lot of single mentions. Now the value of the rank is meaningless.
You can't compare with the ranks of the precoded items.
But - of course - your survey.

If you are interested in this, please send a lss export of your survey (only these relevant questions).

Or you use a "ranking" question.
And open the additional question by relevance, if the "other" is ranked.

I am also confused by this question, just like Joffm.

The question asks people to "rank" up to 5 options (out of 6 total options???) and the question type is a multipunch question.

How can you rank via a multipunch question? You can chose up to 5 that are important, but I wouldn't call this a "ranking".

Are you actually trying to add a free text option to an array rating question as it was shown at Joffm's screenshot?
