listquestiongroups broken?

Please help us help you and fill where relevant:
Your LimeSurvey version: LimeSurvey Community Edition Version 5.6.68+240625
Own server or LimeSurvey hosting: Own Server, LAMPP
Survey theme/template: Default

Hi Community,
it seems, that the group listing is broken at our installation.
The Link shows 404:

myserver.de/limesurvey/index.php/admin/survey/sa/listquestiongroups/surveyid/XXXXXXX

It also shows up totally different from the listing of the questions:

myserver.de/limesurvey/index.php/questionAdministration/listQuestions?surveyid=XXXXXXX

Is this a bug? Did any upgrade break the code?

The question list and question group list move to the same url,
with tab
 

You can reset the menu item to delete old url.

How can I reset the menu?
The first one on the picture "Fragen auflisten" (list questions) leads to the URL:
/limesurvey/index.php/questionAdministration/listQuestions?surveyid=XXXXX
The second one "Gruppenliste" (list question groups) leads to nothing (loading cursor).
Opening in a new tab via right click leads to the URL:
/limesurvey/index.php/admin/survey/sa/listquestiongroups/surveyid/XXXXX
But the result is a 404: "Das System konnte die angeforderte Action "survey" nicht finden."
This problem is within every survey and with different browsers. It seems that this behaviour started with an update not so long ago.
How can I fix this?

OK, now it got worse.
Nearly all the menus have been empty.
I added the Buttons "Fragen auflisten" and "Gruppenliste" to the empty(!) menu "survey settings" but the action for "Gruppenliste" is the same behaviour as I already described (infinite loading cursor, 404 in new tab).

Maybe you need to reset Menu + Menu items : you have 2 buttons.

Sometime i do both 2 times.

We already have issue with some menu not deleted. I need to check what happen and fix it again.
