If you posted in German I would have never found this solution. The solution is simple but the system for this is not intuitive.

The solution for the next person to find this.
You cannot have any extra text in the Subquestion box when using Image select multiple choice. If you need to add text you will need to use a regular Multiple Choice question and an alt img to the Subquestion box like this Apple<br /><img alt="" src="/upload/surveys/379128/images/apple.jpg" /> ​​​​​​

For this to work:

1. Use the URL from the dialog box when browsing the server or
    
   
2. Remove <img alt="" src=" and " /> from the Subquestion
