Are you aware of a repository-like website where a multitude of questionnaires on various topics can be downloaded? Instead of developing a questionnaire fully yourself, it would be handy to find existing questionnaires about eg satisfaction with a restaurant, cinema, shop, etc. and tweak it to one's liking.
