Similar to other uses who have posted about this issue before, when I preview a survey I am unable to move past the welcome screen (the next button essentially does nothing). If I disable the welcome page and begin on the first question, when I try to move forward I get an error that I am navigating incorrectly. 

To solve this issue before other users have had luck either deleting excess HTML accidentally pasted in or by updating to the newest version. However neither of those fixes have worked for me so far. Even after creating a new survey from scratch and keeping the theme as all inherited, the problem persists.

What is strange is that the problem only exists in google chrome. I tried deleting all caches and cookies and then running the survey again but that did not help. Likewise when I run the survey in safari it works fine. Even when I downloaded the survey .lss form from chrome and then uploaded it to safari it worked fine. Let me know if anyone has had prior experience with this, or ideas on how to further troubleshoot.

Edit: After trial and error I found the culprit — the dashlane chrome extension was causing the problem. Does not seem like this issue has been documented before.
