When extended, Bootswatch becomes very problematic
The theme does not seen to save some variation options.
It does not allow images preview: when choosing to preview an image (in the "Theme options" menu), it produces a bad request error (404), "due to malformed syntax".
It does not allow showing any logo, even if the relevant file is explicitly defined in the Custom.css file. On this point, the theme acts as if the option "Show logo" is off.
This behaviour has been tested in two different configurations (see attached images).

Thanks for your reply. I will try to describe in a better way the problem, which appears also with the theme you suggested (Skelvanilla).
Method 1
Extend the theme (either Bootswatch or Skelvanilla)
Select extends_bootswatch (or extends_Skelvanilla with Flatly base variation).
In "Themes", under "Theme options" of either theme try to select a logo file and save the option. --> In both my installations the logo file field remains blank after saving, no matter which file I select, and no matter how many times I press "Save".
The option is not saved and no logo appears in the theme editor (neither in any survey with this theme when previewed or executed).
Method 2
Create a test survey
Select extends_bootswatch (or extends_Skelvanilla with Flatly base variation).
In Settings/General settings select extends_bootswatch (or extends_Skelvanilla with Flatly base variation).
In Settings/Theme options set Brand logo (and/or Use logo as banner in extends_Skelvanilla), and select the Brand logo file.
Press "Save" at the top of the page. --> In both my installations the field remains blank after saving, no matter which file I select, and no matter how many times I press "Save".
No matter what I set in the options, the theme does not show any logo whatsoever. Very frustrating.

Sorry, I missed to add that you have to choose a logo file other than the default one. Both themes are supposed to allow for logos other than the default one. So, upload another logo file (keeping the original specifications) and try to select it as logo.
I also found out that the options adding menu choices in the header of the survey (e.g. "Save" or "Load" an incomplete survey, etc.), affect the logo display.

Is it possible that there is some error in both my installations? E.g. could this be a problem related to the PHP settings on the server? I am surprised that these issues seem to appear only in my (two different) installations (and servers).
Information, to save a value for logo (on extended global theme option) i need to
1. Upload image
2. Reload theme option
3. Set Yes to logo
4. Select the image
5. Save
After select the extended theme for a survey : the logo is OK after the second launch if debug = 2 (1st time i have an issue)

I followed these instructions. Unfortunately, no result. The theme keeps ignoring the setting and acts as if the brand logo setting is off. Screenshot 1 is from theme options-global and screenshot 2 from theme options-survey. No matter what I do, no image setting is saved.
Besides, each time I try to upload an image using the "Upload" option found in the survey theme options, I get the message:
"This file is not a supported image format - only the following ones are allowed: GIF,ICO,JPG,PNG"
I am guessing that there is something wrong either with some configuration files of this installation or with some PHP settings.

Thanks! It worked fine. the fileinfo.dll was not enabled.
It's really great if you can report the issue ?
My opinion : we need a better error returned ("No way to check image file")
