Hi,
Hoping someone can help. 

Previously my bootstrap theme looked like this:
 

Now my bootstrap theme looks like this:
 

Does anyone know why this has changed? or can help me change it back?

We haven't made any recent changes to the survey or the theme and it has changed by itself. We think it is displaying mobile mode but on desktop. Please does anyone have an idea as to why this is happening?

Can you provide a sample survey (.lss file) containing only the relevant question?
