Multiple Choice + Comments: is it possible to divide the comment box in three?

Dear all,

I'd like to use the "Multiple Choice with Comments" question as follows:

Please select which of the following antidepressants you have taken. If possible, enter the dose, frequency and period of use in the field next to it. Multiple selections are possible.

Example: Citalopram, 1 x 20mg daily, 2017-2019


I guess it is much easier for participants to adhere to the structure above, if there are three comment boxes/fields, one for the name (citalopram), one for the dose (1x 20mg daily) and one for the period of time (2017-2019).
Anyone who could help me?

You should consider a different question type.
E.g. array(text), maybe with drop-downs.

Just to consider:
If you allow an open answer you will despair as you encode the entries.
That's the way of a paper-pencil-questionnaire.
Here you should structure your answer options.
Something like this?
By the way: I used bootstrap buttons in the first question only to save space for a better screenshot.
