Sequence numbering of questions in group

Sequence numbering of questions in group was created by mbarbalas20
I am working on a survey and having trouble with relatively simple questions (yes/no). Sometimes they will display with a circled red X in the Question Code field (even though the code is a unique sequence). When I looked at the question Group and looked at the list of questions, I saw questions with the same sequence number.

How did this happen and how to get the sequencing to correct itself?

I am running Version 4.1.16+200407
Running on Windows 10/Firefox browser version 75.0 (64 bit)

Try previewing the survey and submit at least one screen.

Tried previewing the entire survey and completing each page. Made no difference to the listing of questions within the group. The sequence numbering is still as before.

I did not activate the survey.

When I try to open one of the questions that have shared sequence numbers, the question detail does not load (see screenshot).

All I can suggest is deleting those questions and recreating them.
