Lime not always storing answers?

In a certain cases, in my survey I'm finding that Lime doesn't store an answer that was provided by the user. Does anyone else have this issue if so why? Also, I've had when the question was marked mandatory.

What kind of question?
Are you certain the user did submit the results of the survey?

Yes, we are sure they answered. They were two array type questions.

Did you manipulate the questions with JavaScript or are any other scripts applied?

Can you attach a sample survey containing only those questions?

Images don't help much, I meant attach a survey export (.lss) file.

Having said that, those appear to be list-radio questions and not arrays and their relevance equations have errors indicated.

Isn't that normal when it comes to tokenattributes?

Sometimes the error checking is showing red at first and after reloading it's green.

I still would not be so sure that the question was shown.
A limesurvey export will help.

Even if there is no error, from what I see of the relevance equations, the two questions can never be seen together. The relevance equations seem to be exclusive.
