Hi - Few respondents have reported that they are getting below(attached) screen when taking survey . Any ideas what may be the reason and any workaround.

That does not really help without the original survey link to see if the the server is an issue.
Is the survey run on https? It is now contemporary to do that as soon any data is expected to be entered by the user. Otherwise many tools will warn.
