That appears to be a modified template. We have no way of answering the question without seeing a live sample survey.

  • Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
  • Always provide a LSS file (not LSQ or LSG).

I made a new survey. It was much easier...

Try adding this to the end of template.css:
