Again, I cannot reproduce the problem.

Do you have text in the group name and description fields for all languages?

Have you tested with the core default template?

The instructions look like this:




I added them manually on the first question (the one that asks if you understand the instructions), but they should be appearing on every page automatically, as they are on the description part of the question group.
If you look at the first page after the comprehension question you see it's not there:



Is this not what you are seeing? The instructions should be appearing right above every image, but (at least for me) they are not showing up.

When testing with the default template in Version 2.73.0+171219, I see your group title and description in every question screen.

Please update to the latest LimeSurvey version test with the default template.
