Your lss file contained: "How do you like your state? {TOKEN:state}" which gave an error. After correction to ..{TOKEN:ATTRIBUTE_4} all was well (2.06 lts build 160129).
It is important when testing your survey repeatedly as an admin to add "/newtest/Y" to your survey url, like:
"[your.limesurvey.system]/index.php/survey/index/sid/353156/token/Pgg75Rp5mYiPrgX/lang/en/newtest/Y" (this is done by LimeSurvey when you use the "Do survey" icon in the token table.); Especially after making changes to your survey. Also clearing your browser cache might help.

use the "Do survey" icon in the token table
