blank when action in token management

blank when action in token management was created by Dany01
when I go in admin page/list surveys/(choose any survey)/token management/display tokens/(choose any token)... then I clic on the pen and evey icon on this line goes blank.
does'nt matter if I'm in IE or Firefox or Chrome.
Version 2.50+ Build 160512 on windows 2012
apache 2.4.20 VC 14
php 5.6.20 VC11
