in our survey there should be logo of our company in the upper corner (please see picture in attachment). But instead there is a blank icon. Can you advise me, what can be the cause and how can I fix it?

That indicates that the path to the image is wrong or the image doesn't exist.

That would depend on the template being used. It's probably in startpage.pstpl.
