How can I tell if an active survey has been activated in open-access or closed-access mode?  
Can I alter the mode of an active survey to closed-access mode?

Just create a participant table.

No, even though there is a participants table, it's still open to the public. Users can access it, respond, but it fails on submission with 403 error. I'd like for it to be as when activated in closed access mode - you need to provide a valid token first.

And to be able to check which mode is used on an active survey.

I understood that the survey was activated in open mode.

So, when you clicked "survey participants" you saw
 
and after clicking "closed mode"
 

Now you entered your participants, either manually or by uploading a file
 

Created a token, either manually of by generating it.

And when you run it you are asked for the token.

If this is not the case in your survey, please show step by step what you did.

BTW: To check if it is open or closed, use ExpressionScript and see if there is something in the paricipant table.

It used to look like you presented.
Currently, when I click "Participants" I simply see zero participants (screenshot 1). Then when I add a participant, there simply is 1 participant (Screenshot 2). There is no screen about survey participants not initalized, and no choice to switch to closed-access.
