The first is a normal semantic differential with some css in the left column.

The second uses additional headers.

If this is what you want, send the lss export of the survey (only the relevant questions)

This is exactly what we want (except little differences)

We just  decided a different layout for the other question so this is the only one we need.
Please find attached the lss file. 

Can you teach me how to do it or is not possible?

You sent a normal array, but not in semantic differential layout.
