Creating a nested array with multiple subquestions

Hello,

First of all, I'm new to the forum so if i miss out on any information that is needed please forgive me. 

I want to create an question with nested sub-questions in LimeSurvey where the last column should include a radio-button on each row.

I have tried to examplify what I intend to do in the attached picture. I have tried to create a table in an array question and then control the radio-buttons through the JavaScript in the question. However, I only managed to add a radio-button to the first row in column D and not all of them. In Column C the respondent should be able to give an numerical input. 

Once again sorry if my information is insufficient and thanks in advance for any help!

Why a radio button? Once that is selected, it cannot be deselect by the respondent

Thank you both for responding! 

Attached is the LSS-structure file. Note that I haven't added the numerical input. My thought was to first fix the buttons and then add the text inputs.

And regarding the choice of Radio-button. Perhaps its not the best idea. I need it to be the exclusive option for each row.

    Hi,
    wy do you use a question of type "array"?
    You want to enter numerical values in "minutes".
    So at least it should be an array(numbers) or an array(text)

    And you use this HTML table layout that is usually used to design a conjoint experiment. And again you only insert radio-buttons, but no text fields.

    Hi Joffm,

    Well the honest answer question is I'm not sure (due to my lack of knowledge). I copied my question from a colleague's survey and tried to restructure it to the way i wanted. The colleague who wrote the original question is currently on vacation and that's why I turned to this forum. 

    I will go through your examples! Thank you so much for all the help!
