Please help us help you and fill where relevant:
Your LimeSurvey version: [see right hand bottom of your LimeSurvey admin screen]
Own server or LimeSurvey hosting: LimeSurvey hosting
Survey theme/template:

=10ptI use the “Text display" to create a question.=10pt I add the statement in the description box to listen to the audio clip and upload an audio file using the “Insert HTML5 video” function into the question description box (see upload audiofile screenshot attached).=10pt However, when I preview the question or the survey, respondents see only the comment to listen to the audio file but the link to the audio file does not appear. It seems that the link is not saved in my question for some reason because when I return to the question to edit, the link I created is not there anymore (see Survey preview screenshot attached). How can I upload an audio/video file, so respondents can view it? Does the admin need to change the Global Setting before I can upload audio/video files? Thank you for responding.

And this is the part you didn't show use.
Looks as if you still insert the <video> tags.

I do not see an issue with

Tried changing video to audio using your code, but when I save the question, the code disappears and audio file is not shown to the respondent.

The only thing to do:
put the HTML-code into the source code of your question.
Make sure you enter the correct path.

And do you have rights to insert this?
Are you a superadmin or a regular user?
In this case ask the admin.

I did try entering the HTML-code. I think the problem might be related to admin rights. I will reach out to the admin. Thank you for you help!

Most probably the XSS-Filter is on and you are not a superadmin (the XSS filter doesn't impact superadmins).

So get in touch with the person responsible for your LS installation.

Help us to help you!

Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
Always provide a LSS file (not LSQ or LSG).

A few months ago me and some other developers tried workin on a new Limesurvey question theme to store recorded audio files. We got it working but it was quite some hassle.

Let me know if I should provide the question theme for further testing.
