I have to design questions with the design shown in this picture:


So first question: How can I add another column (since the default two arrays design - current and past - is not enough)?

Second: How can I change the "answer type" of a column (for the first 2 there should be the choice between yes and no but for the last one I need a numerical input)

Another question I had is: how can I select which answers are mutually exclusive? For example when choosing "no" for Q1 in the "current" column, can I automaticly disable Q2 and Q3. Ive only seen these conditions to enable / disable an entire question but not some of its subquestions.
In addition to that: Is it possible to have e.g. Q1 as a "single choice" and Q2 as a "multiple choice" answer?
