Multiple columns for text display in an Array

Hello Forum Members,

I am trying to create the below format in an array with just 1 scale dropdown column. Is it possible to create 
Basically the client want to have responses for the Level attribute. The others columns are just for information.

did you consider how it can be answered on a smartphone?
How to display this?
Always remember: This is not a paper-pencil survey, but an online survey.

Nevertheless, some ideas:
1. If it is "just for information" use a "text display" and then display the rating question. You may merge the questions visually by css.
2. Create tables in the subquestion text
3. Here two threads where radios are moved into the table that is created in the question text.

4. Question text and answer options side by side (image = your "information" table, buttons = your drop´down)
 
5. As "Carousel" question. After selecting the rating the next one is displayed (some libraries used "tiny slider", "slick")
