I noticed that when uploading an image to KCFiner, then deleting it, then uploading a different image with the same name can keep the original picture when referred to in, for example, the question text. The thumbnail however is the updated one. I do not know whether this is always the case or if the server just takes some time to update. Changing the name helps. However, in my specific case this is not really possible and I really need to be sure that the pictures update (at least after a while).
This shouldn't be used in production or being online anymore. This is a 5,5 year old installation without having received any patches, bug and security fixes for the same amount of time and in addition isn't even the latest release of its branch (which I consider the worst release of Limesurvey and I would never have applied it to any productive environment. In my opinion this branch of Limesurvey was never really stable, for my taste. I don't think this should be used for collecting data in 2022.

Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
Always provide a LSS file (not LSQ or LSG).
