Thanks a lot for this. Would it be possible to use an Array(numerical input) with checkbox Q. style instead of Array (text) for the sub-question Q.2? Currently the unique names are fed into the first answer column of the array text question, but for an array with checkbox layout the names would need to feed into the Y sub-question scale? 

I would want to do something like this:

Q.1. Which org are responsible for the following areas?
Area 1. *Name of organisation 1*
Area 2: *Name of organisation 2*
Area 3: *Name of organisation 1*
Area 4: *Name of organisation 2*

Q.2. Please check the box if the following statements are true:  

Column 1(The org has X in place) Column 2 (The org has Y in place) Column 3 (The org has Z in place) Column 4 (The org is in group W)

Would this work? Alternatively if the array with checkbox layout is too complicated multiple follow up array-style sub-questions would also suffice...
