Oh,
I did not see any script in your lss. Nor did you create the - for this solution - necessary following text questions.

This thread was about an array with some text fields BEFORE the checkboxes.
Now you show that you want the text fields AFTER the checkboxes.
This is a different thing.
You may adapt this script
