I am searching for a way to implement an array in my survey. It should be similar to the one attached. Except, I would like to rename the columns as well (instead fo the scale 1-6). 
By using the array(text), one is able to rename the rows and columns. But I would like to let the respondent tick the options. So that didn't work. Is there another function I am accidentally missing?
Beyond that I am thinking about adding one more row in the end, where respondents can add one new bullet, which is missing in the enumeration. After they added that item, they should also be able to select the applicable boxes.
