I am trying to combine several question types into one. It should look like this:
 
It has 5 columns and several rows(maybe with the option to hide extra rows and only show them if all above are filled). The columns should display the following:
1) text input (the title of a seminar or a short description)
2) dropdown with 2 options (mandatory if a text input is given)
3) numeric input (credit points of the seminar)
4) dropdown with 2 options (mandatory if a numeric input is given)
5) dropdown with 2 options (mandatory if a numeric input is given)

I am thankful for any help or advice.
