capture reason for opt out

capture reason for opt out was created by dAdapt
I have a existing site that I need to change to now be able to capture an opt out reason in text and a drop down list.
Can you tell me how I can capture a reason for opt out?

Maybe I am missing something, but I can't see anything that can get a reason when they click the opt out link.

I think the OP refers to when the opt-out link is clicked in the invitation email.

This is how I would behave as a recipient of an email.
 
Normally, I would simply ignore or delete such an invitation email for a survey.
It's an extra hassle to click the "OptOut" link to say "Damn it, I don't want to take any polls or get any more emails from you or your company".

What do you think I will feel if this is ignored and a survey is started afterwards.
Even if it's just the question "Why not?"

On every welcome page you will find a phrase like this. "Participation in the survey is voluntary. You can end the survey at any time without giving a reason."
These are cornerstones of market and opinion research: "Voluntariness" and "Refusal without giving reasons"

Anyway: the answer to your last question about this 5 years ago (jelo's link) is still valid, 
But feel free  to write in the invitation mail "If you opted out, would you please send an email with the reason for that"
