This is an array question with the two answer options separated via "|" to present them on both sides.

You can define your codes yourself, but what could be problematic is the display the scale from 9 to 9 in different colors. For that you will need to use some custom code via CSS I guess.

You should have a look into the manual, where each question type is described in more detail, because a question type can have different layouts:
www.limesurvey.org/manual/question_types

www.limesurvey.org/manual/Question_type_-_Array

Look for the part with "semantic diferential"

As @holch: Read about the available question types.

Well, yes it is possible, more or less out of the box (you only give a additional style to the red numbers),

But I hope the items are as short as shown.

And I also hope it is not answered on smartphones.

PS.
Do you really think people can answer this question using this scale?
I can't imagine that a respondent is able to distinguish between 3 and 4.
Having said this: you may also consider to use a slider.

thank you very much for your quick response. I have already made one. But there is a problem with the width of the sub-question as it seems from the screenshot below.
How can i fix that???
thank you in advance

Read the manual about question types (especially "array")

I agree with Joffm, sliders would be far more user-friendly on mobile devices.
