Dual array with forced embedded comments?

Like many others, my university had to move their courses online on short notice recently. We are trying to run some evaluations across Canadian universities to help professors give and share feedback about what is working for them.

I am putting together a survey in which one question will ask them to evaluate different virtual learning web conferencing options. I want to list a number of software packages (eg. Adobe connect, webex, zoom), ask faculty if they have used them, if they would recommend them, and force them to give a comment on each package that they have used. It would essentially be three columns. I can't figure out how to make two columns yes/no then force the comment for each software that they have used.

I also want to give some "Other" spaces at the bottom of the list, and allow people to add the names of other software packages that are not on the list.

Any thoughts on how I can accomplish this?

Version of Lime is Version 3.22.11+200330, so this should work.
