I want to do something like that in Limesurvey:

A patient comes to my clinic, that person fills out a standard survey:
- Name
- Phone
- and some personal information

Then, one of my workers takes that survey and that worker continues to fill out other information about that patient, such as "Test to be performed" and the different data needed for that test.

Then another worker takes that survey and fills out "medical test results" and other data in addition to those results

Is there a function or plug-in to do something like this?

Currently what I do is make a survey with 6 sections, only 2 are seen by the public and my coworkers from the administrator edits the answers of that person from the "edit answers" function. Unfortunately, that function has a bug in LimeSurvey 4 and that's why I'm looking for an alternative to do it

My opinion :

1. Don't use 4.X currently, better to use 3.X
2. Try gitlab.com/SondagesPro/SurveyAccess/surveyChaining

Are the different people using the same computer/tablet or different devices?
If yes, you could work with End-URL to forward them to another survey.
If not, you could try to use confirmation/notification emails with links to the next survey.
But both depends a bit on your workflow

My ideas are probably not the best ones (both ways won't use the SAME survey for the different people, but I don't see the advantage of that), but these would be my solutions if I only use the built-in functions.

Solution with a single survey (on multiple or single devices)...

• Use tokens.
• Activate "Allow multiple responses" for the tokens.
• Add a hidden question and relevance for the coworkers groups based on that question.
• The patient enters the survey with a normal URL.
• The coworkers enter the survey with a parameter appended to the URL that prefills the hidden question, showing their question groups.
