I agree with Joffm, this looks pretty much like the "semantic differential" layout he describes. Have a good look at Joffm's link and what it shows about "semantic differential".
If this really isn't what you are looking for, we need a better description, because it seems that everyone (Joffm, Tpartner, me) seem to understand it in the same way.

Provide your LS version and where it is installed (own server, uni/employer, SaaS hosting, etc.).
Always provide a LSS file (not LSQ or LSG).

Thanks, this semantic differential (whoever came up with that name) was really what I was looking for - I would have called it a two sided Likert question.
