I Would like receive some help with other tricky thing, you are the experts, I wish you can help me with this, thanks in advanced.

I have a multiple short question with three boxes and i need to have a fourth options that be a radio button that disable the multiple short question. Like an exclusive option.

I did this (attached picture 1) but we need something like this (attached picture 2).

How can i do this?

checkBoxForText is not on LimeStore curently (and clearly in test for 3.X version : work, but must be improved).
