I have two quick questions:

1) Which code would I use to insert an email address into hyperlink at the end of the survey. Respondents will be directed to a contact person if they wish to be entered into a draw.

2) Which code would I use in order to change column lengths, such that the questions column (far left) is wider than the answer columns. Please see the screen shot. Thank you.

1) You can insert mailto links in survey elements (questions, group descriptions, end message...) with the HTML editor.
